EVENT SPECIFIC REQUIREMENTS
World Trade Center

This requirement is only for people selling on Thursday or Fridays at the World Trade Center.
1. FOOD SAFETY QUESTIONNAIRE
You must complete the linked word doc. Please type a response to each of the sections and return. You cannot leave any questions blank, or type “yes” as an answer. Each question needs an answer in the form of a full sentence.

2. HOLD HARMLESS AGREEMENT
This needs to be signed and both pages returned as one document. If you send just the signature page back we’ll just have to ask you to do it again. Pls return the full document.

3. INSURANCE
See the linked sample COI that you should replicate as closely as you can via your insurance broker or agent. Please send the sample to them. The certificate(s) would need to include/show your automobile insurance (with the additional insureds included) and workers compensation coverage. It needs to also include any specific check boxes and text included in the sample, so please send the sample direct to your broker or agent and ask them to copy it.

Notes: We do not need separate COIs for all the listed additional insured, but we do need the names added as in p.2 of the sample cert.
You do need proof of auto insurance, and you do need a workers compensation policy if you have anybody working in your booth who is not an official owner of the company. Please talk to your insurance agents.

Health Department Requirement
The Health Department works in conjunction with the Department of Culture Affairs to issue food licenses. There is a $25 per event fee for a special event food license. Most vendors collect permits monthly, in-person in Jersey City.
A Food Service Establishment permit (or Jersey food permit) for vendor's own restaurant or commissary/kitchen OR or the permit/license issued to a shared kitchen, along with a contract/agreement that proves you cook in that shared kitchen. Ideally these permits are unexpired, but we have received confirmation from NYC DOH that permits expiring after 3/12/20 are still valid indefinitely under executive order from the Mayor. Please send the most recent you have for our files.
If you work only using TFSEs, please send the most recent TFSE you have. Ideally you have a new TFSE, but we have received guidance from NYC DOH that TFSEs expiring after 3/12/20 are valid indefinitely by executive order of the Mayor. People permitted in NJ will still need a special event permit for this location.
Copy of most recent inspection report for your restaurant or kitchen space from the relevant Health Department. Yes, this is required from everyone, including from shared kitchens.
Copy of a valid driver’s license for ID purposes
Copy of NJ or out-of-state business registration certificate. This means your tax ID. This does not have to be in NJ. You can just send a copy of your federal Tax ID (Certificate of Authority), or even your Social Security card if you are not incorporated.
Copy of Food Protection Certificate OR Serv-Safe Food Manager or Food Handler for all employees selling the product for the vendor. We may have one on file for you and we’ll use them, but we actually need as many as you can produce from your team as they ideally want anyone/everyone in the tent to have one. Please just send as many as you have access to.
Complete the attached “special events food vendor form”. Use your business name connected to your permits as well as a DBA if applicable
Fire Department Requirements
Cooking Permit for regular stands is a $125 one-time payment for the entire year.
